Clinical waste disposal costs UK: calculating what practices pay for collection and containers

Clinical Waste Disposal Costs UK 2027: What Practices Actually Pay

In the UK, clinical waste collection for a small practice usually costs a fixed visit or consignment charge plus a price per bag or container. Published 2026/27 council trade rates start at about £7 to £18 per sack or sharps box plus a visit charge of around £10 to £23. One private provider, Priority Waste, lists from £9.50 per clinical waste bag and £24 per 50-litre box, plus a consignment note fee of about £56 per collection. Vena Cava's online one-off booking is a £119 visit (consignment note included) plus £8.14 to £32.30 per sharps bin, ex VAT. Every price below comes from a published price list checked on 31 October 2026, and each source is named so you can check it yourself.

Clinical waste is one of the few overheads where small practices pay very different prices for the same service. Quotes are often given only over the phone, and they bundle visit fees, container charges, paperwork fees and surcharges in different ways. This guide pulls the publicly listed prices together, explains what each part of a quote covers, and works through what a GP surgery, dental practice, aesthetics clinic, care home and tattoo studio might pay.

How much does clinical waste collection cost in the UK?

There is no national price list, but the published prices fall into a few clear bands. The table below summarises them by pricing unit. All figures are as published by the named source, and the VAT treatment is shown where the source states it.

Price element Published range Where the figures come from
Visit or collection charge £9.71 to £119 per visit Royal Borough of Greenwich 2026/27 (£9.71), North Norfolk District Council (£23), Vena Cava online booking (£119, includes consignment note)
Per infectious clinical waste sack or bag £7.20 to £18.01 per sack (2026/27 council lists); £80 per sack including collection in one 2025/26 list; from £9.50 per bag (private) North East Derbyshire, Royal Greenwich, South Derbyshire 2026/27; North Warwickshire 2025/26; Priority Waste
Per sharps bin or box £7.54 to £32.30 per container Royal Greenwich 2026/27 (£7.54), Kensington and Chelsea 2026/27 (£12.20), Vena Cava booking (£8.14 to £32.30 by size and lid colour)
Offensive (tiger) waste £3.20 to £3.80 per sack (council) South Derbyshire 2026/27 (£3.20 hygiene sack); North Norfolk (£2.50 sack plus £1.30 disposal)
Hazardous waste consignment note fee £0 (included) to £56.60 per collection Vena Cava (included in visit fee); Priority Waste (£56.50 to £56.60)
Small-practice packages "From £100" (solo) and "from £175" (two or more practitioners) Sharpsmart aesthetics clinic packages, quarterly collection; the billing period is not stated on the page
Annual contract equivalent From £1.14 per day (about £416 a year); £570 per weekly sack per year (council) Affordable Waste Management (starting price); Derbyshire Dales District Council (plus VAT)
Third-party guide estimate £40 to £150+ per pickup BusinessPrices.co.uk 2026 guide, for hazardous and clinical waste combined

The single biggest swing factor is the fixed charge per visit. A council with a £9.71 visit charge and a private one-off service with a £119 visit charge can both be good value, depending on how many containers you put out each time and how often you need a collection. That is why comparing quotes on the per-container price alone gives the wrong answer.

What do councils charge for commercial clinical waste?

Some local councils still collect clinical waste from businesses as part of their trade waste service, and they publish their charges in annual fees and charges schedules. Many do not. Reading Borough Council, Mid Sussex District Council and the London Borough of Bexley each state on their websites that they do not collect commercial clinical waste, and North Norfolk District Council stopped collecting infectious (Group A and B) clinical waste from commercial premises in April 2023 and is not taking new commercial clinical customers. Where a council does offer the service, these are the published rates.

Council (year) Published charge Unit and notes
Royal Borough of Greenwich (2026/27) £9.71 visit charge; £7.54 per sack or sharps box Commercial clinical waste. Up from £9.35 and £7.26 in 2025/26. VAT not stated
Royal Borough of Kensington and Chelsea (2026/27 draft schedule) £37.30 clinical waste collection (except sharps); £12.20 sharps collection; £12.20 sharps drop-off Listed under pest control, excluding VAT. Unit not stated, and the schedule does not say whether these are domestic or commercial charges
South Derbyshire District Council (2026/27) Clinical with disposal: £18.01 per sack; £52.75 (240L) to £237.19 (1100L) per bin Per week. "VAT will be charged where applicable"
South Derbyshire District Council (2026/27) Hygiene: £3.20 per sack; £9.61 (240L) to £31.70 (1100L) per bin Per week. Hygiene waste is non-infectious, similar to the offensive stream
North East Derbyshire District Council (2026/27) £9.44 per clinical sack (commercial); £7.20 (schools and care homes) 3.16% increase on 2025/26. VAT not stated
Derbyshire Dales District Council (current page) £570.00 plus VAT per sack per year Category A clinical waste, one sack collected each week
North Warwickshire Borough Council (2025/26) £80.00 per sack (3.5kg capacity), including collection Shown as outside the scope of VAT
North Norfolk District Council (year not stated) £23 per visit; £2.50 per sack delivered; £1.30 per sack disposed of Group E (offensive) waste only, such as incontinence pads

Two things stand out. First, council prices vary by a factor of ten for what looks like the same sack. North Warwickshire's £80 per sack includes collection, while Greenwich charges a separate visit fee. Second, council services are local, often limited to certain waste streams, and may not issue the full range of containers a practice needs (purple-lid cytotoxic bins, pharmaceutical bins or amalgam pots). Check what your council covers before assuming it is the cheapest route.

What do private clinical waste companies charge?

Most national and regional clinical waste companies do not publish prices. They quote after asking for your waste streams, volumes and postcode. The few public figures we found are below. Several companies advertise offers instead of prices, such as free bins and delivery (Business Waste) or a free first collection or a free twelfth month (Affordable Waste Management).

Provider Published price What is included
Priority Waste From £9.50 per CliniBag; from £24.00 per CliniBox 60L bag holding up to 7kg; 50L box holding up to 15kg. Plus a hazardous waste consignment note fee of £56.60 (bags) or £56.50 (boxes) per collection
Sharpsmart (aesthetics, solo) Prices start from £100 30L single-use sharps bin, clinical waste bags, education materials, quarterly collection of one 30L sharps bin and three bags
Sharpsmart (aesthetics, 2+ practitioners) Prices start from £175 30L sharps bin per person, 360L clinical waste bin and bags, quarterly collection
First Mile (non-medicinal sharps) Pay-as-you-go container collections from £7.55 Free next-working-day box delivery and collection booked online. Container size not stated
Affordable Waste Management From £1.14 per day Starting price for repeat collections; quotes not offered for one-time pickups

Read "from" prices carefully. Sharpsmart's page does not say whether its starting prices are per collection, per quarter or per year, or whether VAT is included. Priority Waste's prices carry an asterisk that the page does not explain. Ask any provider for a written quote that states the unit, the VAT treatment and every additional fee.

What does Vena Cava charge for a one-off collection?

Vena Cava arranges clinical waste collections through licensed waste carriers and permitted treatment facilities, and is registered with the Environment Agency as an upper-tier waste broker and dealer (CBDU669967). Its online booking page shows the full price before you pay. According to the clinical waste collection FAQs, you pay a fixed £119 collection visit fee plus a price for each container. All prices exclude VAT, which is added at 20%.

Item Price (ex VAT)
Collection visit, incl. consignment note £119.00 per booking
Yellow-lid sharps bin 2L / 5L / 13L / 24L £8.14 / £9.19 / £15.28 / £27.67
Purple-lid sharps bin 3L / 5L / 13L / 24L £11.00 / £11.50 / £19.22 / £32.30
Orange clinical waste bags (pack of up to 4) £18.48
Tiger offensive waste bags (pack of up to 4) £14.64
Yellow incineration box 50L £23.36
Blue-lid medicines bin 7L £12.68
Dental amalgam pot £41.38

What the booking page and FAQs say is included: an instant price, no setup fees, account fees or minimum terms, consignment notes included, and a full refund if the pre-acceptance check shows the waste cannot be accepted. Bookings need at least 10 working days' notice, and the date is confirmed by email within 2 working days. The visit fee covers the licensed vehicle and driver and the hazardous waste consignment note, and is charged once per booking however many containers you have.

For regular monthly or quarterly collections, the clinical waste collection service page offers a fixed monthly price quoted from your details or a recent waste invoice, usually within 48 hours, with no multi-year contract. If you only need sharps collected, see the sharps bin collection service.

Do you pay extra for consignment notes and duty of care?

Often, yes. Paperwork is a legal requirement, not an optional extra, but the way it is charged varies:

  • Consignment note fees. Hazardous waste, which includes most infectious clinical waste and sharps, must travel with a hazardous waste consignment note under the Hazardous Waste (England and Wales) Regulations 2005. Priority Waste lists a fee of £56.60 per collection for this. Vena Cava includes it in the £119 visit fee.
  • Environment Agency consignee returns. The site that receives hazardous waste pays the Environment Agency a fee for each consignment it reports: £10 for an electronic return or £19 for a paper return for a single consignment, and £5 or £10 per consignment for a multiple collection round, according to GOV.UK. This is a cost to the disposal site, but it is one reason contractors charge per consignment.
  • Annual duty of care, environmental and fuel charges. Kernow Clinical Waste, a Cornwall and Devon provider, warns that fees described as an "Annual Duty of Care" fee, an "Environmental Surcharge" and a "Fuel Supplement" are often missing from initial quotes. No provider we checked publishes the amounts, so ask for them in writing.
  • Waste transfer notes. Non-hazardous offensive waste moves under a waste transfer note instead. An October 2026 business waste price guide from ExpertSure says most companies include paperwork costs in their service fees.

For what each note must contain and how long to keep it, read our guide Waste Transfer Notes Explained.

How much do sharps bins and clinical waste bags cost to buy?

Containers are the smaller part of the bill, but they are not always free. Some contractors include bins in the collection price (Business Waste and Sharpsmart both say containers are included), while others charge for them or expect you to buy your own. Published council prices give a benchmark: Hull City Council lists yellow clinical sacks at £42.11 for a roll of 200, a small sharps box at £6.63 and a large sharps box at £12.16 (year and VAT treatment not stated). North East Derbyshire lists a box of 200 clinical sacks at £1,830.00, which equals 200 sacks at the £9.15 per-sack rate on the same page, so it is a collection charge rather than the price of the bags.

What drives the cost of clinical waste disposal?

Every quote is built from the same handful of factors. Knowing them lets you compare like with like.

  • Waste stream and treatment route. Under the Department of Health's HTM 07-01, different streams go to different treatment. Orange-stream infectious waste can go to alternative treatment, yellow-stream and medicinally contaminated waste goes to incineration, and purple-stream cytotoxic waste must go to high-temperature incineration. Offensive (tiger) waste is non-hazardous and is the cheapest stream to dispose of. The council lists above show the gap: South Derbyshire charges £18.01 per clinical sack with disposal against £3.20 per hygiene sack.
  • Hazardous or non-hazardous classification. Hazardous waste, for example infectious waste coded 18 01 03* in the List of Wastes, needs a consignment note, which can carry its own fee. Non-hazardous offensive waste (18 01 04) moves under a waste transfer note instead.
  • Volume and container size. Larger containers cost less per litre. On Vena Cava's booking page a 24L yellow-lid bin costs £27.67 to collect, about £1.15 per litre, while a 2L bin costs £8.14, about £4.07 per litre.
  • Collection frequency. Every visit carries a fixed cost. Collecting four bins once a month is cheaper than one bin every week, as long as you can store the waste safely and within your carrier's storage guidance.
  • Location. Rural sites and sites far from a carrier's round usually pay more. Council services are limited to their own area.
  • Contract length and minimum charges. Long contracts may lower the headline rate but lock you in. Fixed visit fees and any minimum charges fall more heavily on low-volume sites, which is why a small practice can pay more per bin than a large one.
  • Surcharges. Fuel supplements, environmental surcharges, annual duty of care fees and excess weight charges are add-ons that Kernow Clinical Waste says are often left out of the initial quote.
  • VAT. Private providers usually add VAT at 20%. Council treatment varies: North Norfolk states that clinical waste from residential homes is outside the scope of VAT while other commercial clinical waste is subject to VAT, and North Warwickshire shows its clinical sack charge as outside the scope.

What does a typical practice pay? Worked examples

These are illustrative estimates, not quotes. They apply the published prices above to an assumed waste profile for each type of setting. Your own volumes, location and provider will change the answer. Prices are ex VAT unless the source states otherwise.

Single-handed GP surgery (monthly collection)

Assumed monthly waste: two 13L yellow-lid sharps bins, up to four orange bags, up to four tiger bags.

  • Vena Cava one-off booking, monthly: £119 visit + (2 × £15.28 = £30.56) + £18.48 + £14.64 = £182.68 per collection, or £2,192.16 a year if booked twelve times.
  • Royal Greenwich council rate, same volume collected monthly: £9.71 visit + (2 sharps boxes + 4 sacks = 6 × £7.54 = £45.24) = £54.95 per collection, or £659.40 a year (× 12). This covers clinical waste only, as Greenwich does not list an offensive waste rate, and is available only to premises in the borough.
  • Priority Waste, bags only, monthly: (4 × £9.50 = £38.00) + £56.60 consignment note fee = £94.60 per collection, or £1,135.20 a year, before any sharps charges.

Dental practice (quarterly collection)

Assumed quarterly waste: one amalgam pot, two 5L yellow-lid sharps bins, up to four orange bags.

  • Vena Cava one-off booking: £119 + £41.38 + (2 × £9.19 = £18.38) + £18.48 = £197.24 per collection, or £788.96 a year (× 4).
  • Amalgam is a specialist stream that many council services do not list. Check that any quote includes it explicitly.

Aesthetics clinic (quarterly collection)

Assumed quarterly waste: one 5L purple-lid bin for botulinum toxin, one 5L yellow-lid bin for filler syringes and needles, up to four tiger bags.

  • Vena Cava one-off booking: £119 + £11.50 + £9.19 + £14.64 = £154.33 per collection, or £617.32 a year (× 4).
  • Sharpsmart solo package: prices start from £100, covering a 30L sharps bin and three clinical waste bags collected quarterly. The page does not state the billing period, so ask before comparing.

Our aesthetic clinic waste collection page explains which bins an aesthetics practice usually needs.

Care home (weekly collection)

Assumed weekly waste: two infectious clinical waste sacks plus offensive waste in a 240L bin, or ten offensive waste sacks.

  • South Derbyshire council rate: (2 × £18.01 = £36.02) + £9.61 for a 240L hygiene bin = £45.63 a week, or £2,372.76 a year (× 52).
  • Derbyshire Dales council rate, clinical sacks only: 2 weekly sacks × £570.00 = £1,140.00 a year plus VAT.
  • North Norfolk council rate, offensive waste only: £23 visit + (10 × (£2.50 + £1.30) = £38.00) = £61.00 a week, or £3,172.00 a year. North Norfolk only offers this for Group E offensive waste.

Care homes produce mostly offensive waste, so moving non-infectious pads and hygiene waste out of the orange stream has more effect on cost here than anywhere else.

Tattoo studio (twice-yearly collection)

Assumed waste: two 5L sharps bins every six months. Ask your carrier which lid colour applies to your waste.

  • Vena Cava one-off booking, yellow-lid bins: £119 + (2 × £9.19 = £18.38) = £137.38 per collection, or £274.76 a year (× 2).
  • Kensington and Chelsea council rate: £12.20 for a sharps collection, ex VAT (unit not stated, and not stated whether it applies to businesses; available only within the borough).
  • First Mile, non-medicinal sharps: pay-as-you-go container collections from £7.55.

Why batching matters with a fixed visit fee

With a £119 visit fee, one 5L yellow-lid bin costs £128.19 to collect (£119 + £9.19). Four of them cost £155.76 (£119 + 4 × £9.19), or £38.94 per bin. Storing full, sealed bins safely and collecting them together is the simplest saving available on a per-visit pricing model.

How can a practice cut clinical waste costs legally?

  1. Segregate offensive waste from infectious waste. Gloves, aprons, couch roll and pads with no infection risk belong in tiger bags, not orange bags. NHS England's Clinical Waste Strategy sets a segregation target of 20% high-temperature incineration, 20% alternative treatment and 60% offensive waste for NHS providers by 2026, and estimates the strategy could save the NHS up to £11 million a year. The same logic applies to a small practice. Our Clinical Waste Colour Codes guide explains each stream.
  2. Do not over-segregate into expensive streams. Putting ordinary medicinally contaminated sharps into purple-lid bins when they are not cytotoxic means paying for high-temperature incineration you do not need. Equally, never downgrade waste that is cytotoxic.
  3. Right-size your bins. Larger containers cost less per litre, provided they fill within your carrier's maximum storage time.
  4. Fill bins to the line, not past it. Collecting half-empty bins wastes money. Overfilled bins are a needlestick risk and may be refused. Close sharps bins at the fill line, and fill bags no more than three-quarters full.
  5. Match frequency to volume. If your bins are routinely half full at collection, move from weekly to fortnightly or monthly, or switch to on-request collections.
  6. Return unused medicines through the right route. Pharmaceutical waste is its own stream with its own container (a blue-lid bin for non-cytotoxic medicines). Keeping medicines out of sharps bins and bags keeps each container correctly described, so it is not rejected or reclassified at collection.
  7. Review your invoice line by line. Ask for any fuel, environmental, duty of care or weight surcharges to be removed or justified, and check your renewal date so you are not rolled onto another long term.

A cheap quote is worthless if it leaves you non-compliant. As the waste producer you keep the legal duty of care under section 34 of the Environmental Protection Act 1990, even after the waste leaves your premises. Any service you pay for should include:

  • A registered waste carrier or broker. Check the registration number on the Environment Agency's public register (Vena Cava's is CBDU669967, upper-tier broker and dealer).
  • A hazardous waste consignment note for hazardous waste, such as sharps and infectious waste, under the Hazardous Waste (England and Wales) Regulations 2005. GOV.UK states that producers must keep their records for 3 years at the premises that produced the waste.
  • A waste transfer note for non-hazardous waste, such as offensive waste, kept for at least 2 years.
  • Correct description and coding of each waste stream using List of Wastes (EWC) codes, so it goes to a facility permitted to treat it.
  • Treatment at a permitted facility, matched to the stream: alternative treatment or incineration for infectious waste, high-temperature incineration for cytotoxic waste.
  • UN-approved, correctly labelled containers. Sharps bins should meet BS EN ISO 23907-1, and the Health and Safety (Sharp Instruments in Healthcare) Regulations 2013 require employers to place secure containers and instructions for safe disposal of medical sharps close to the work area.

What are the red flags in a clinical waste quote?

  • No unit on the price. "From £100" means nothing until you know whether it is per collection, per month or per year, and whether it includes VAT.
  • Paperwork charged separately without being disclosed. A consignment note fee of £56.60 per collection, the published figure from one provider, can be more than the waste itself for a small practice.
  • Unnamed surcharges. Fuel supplements, environmental surcharges and annual duty of care fees that appear only on the first invoice.
  • Low weight allowances. Kernow Clinical Waste warns that a quote built on a low weight allowance can lead to excess weight charges once heavy sharps bins are collected.
  • Long contracts with no exit. The same provider warns of three to five year terms where missed collections are not accepted as grounds to cancel. Check the notice period and any automatic renewal clause.
  • No carrier registration number on the quote or the paperwork.
  • A price that seems too low for the streams listed. Ask where the waste is treated. Illegal disposal remains your problem as the producer.

Methodology and sources

All prices were collected from publicly available web pages and published council fees and charges documents, and checked on 31 October 2026. We recorded each price exactly as published, together with its unit, VAT treatment and inclusions where the source stated them. Where a source did not state the unit, year or VAT treatment, we say so in the tables. We did not use prices given only in sales calls or quotes. Worked examples are our own arithmetic using these published figures and assumed waste volumes. Prices change, often every April for councils, so check the current figure with the provider before relying on it.

Frequently asked questions

How much does clinical waste collection cost per month for a small practice?

It depends on streams, volume and frequency. Using published prices, Affordable Waste Management's starting rate of £1.14 per day is about £35 a month, while a monthly Vena Cava one-off collection of two 13L sharps bins, orange bags and tiger bags works out at £182.68 ex VAT. A Royal Greenwich council collection of the same clinical waste would be £54.95 a month.

How much does it cost to have a sharps bin collected?

Published per-container prices range from £7.54 per sharps box (Royal Greenwich 2026/27 council rate, plus a £9.71 visit charge) to £32.30 for a 24L purple-lid bin on Vena Cava's booking page, plus a £119 visit fee that includes the consignment note. The total depends mostly on how many bins you collect per visit.

Do councils collect clinical waste from businesses?

Some do and many do not. Royal Greenwich, South Derbyshire and North East Derbyshire publish 2026/27 commercial clinical waste rates, while Reading, Mid Sussex and Bexley state they do not offer a commercial clinical waste service.

Is there a fee for the consignment note?

It depends on the provider. Priority Waste lists a hazardous waste consignment note fee of £56.60 per collection, while Vena Cava includes the consignment note in its £119 visit fee. Always ask whether paperwork is included.

Is offensive (tiger bag) waste cheaper than clinical waste?

Yes. Offensive waste is non-hazardous and does not need a hazardous waste consignment note. South Derbyshire's 2026/27 rates show £3.20 per hygiene sack against £18.01 per clinical sack with disposal.

Is VAT charged on clinical waste collection?

Private providers usually add VAT at 20%; Vena Cava's prices exclude VAT, which is added at 20%. Council treatment varies, and North Norfolk states that clinical waste from residential homes is outside the scope of VAT while other commercial clinical waste is subject to VAT.

How can I reduce my clinical waste bill?

Move non-infectious items into tiger bags, use larger bins that still fill within your storage limits, collect several containers per visit, and check invoices for surcharges and auto-renewals.

How long do I need to keep clinical waste paperwork?
